Do you have a passion for making a real difference in the lives of people living with mental health challenges and disabilities At Easterseals PORT Health were seeking a compassionate and dedicated fulltime Mobile Prevention Coordinator to join our team in Northern Virginia. As a valued member of our collaborative and supportive team you will work together to transform the lives of individuals by providing a comprehensive range of services that treat the whole person and empower individuals to find their way forward.
What Youll Do
The Mobile Prevention Coordinator will deliver clinical and coordination services for the REACH Mobile Prevention Team. These prevention services are typically noncrisis in nature and take place in individuals homes or community settings. Key responsibilities include providing onsite and remote (telephonic/telehealth) crisis prevention and intervention services conducting observations making recommendations providing education and developing safety plans. This role will include local travel to our Crisis Therapeutic Home (CTH) and the community: Arlington Alexandria Fairfax Loudon and Prince William counties. (Candidates must reside or be willing to relocate to one of the listed areas). There role also participates in an on occasional oncall rotation which can require onsite response.
*$2000 sign on and retention bonus*
