Job Description

The SCOPE Nurse Navigator serves as the central access point for primary care providers (PCPs) needing to connect patients to appropriate hospital community and specialist resources. As part of the Seamless Care Optimizing the Patient Experience (SCOPE) program this role supports timely care navigation improves system efficiency strengthens care transitions and contributes to reducing unnecessary emergency department visits and hospital readmissions. The Nurse Navigator collaborates with interdisciplinary teams and external partners to optimize integrated patient care pathways.

Key Responsibilities:

  • Serve as a central point of contact for PCPs triaging referrals and connecting patients to hospitalbased and community resources.
  • Clarify and understand primary care provider concerns to ensure appropriate care coordination and service navigation.
  • Navigate hospital clinics specialists and community agencies to support complex patient care needs.
  • Facilitate internal medicine consults and diagnostic followup in collaboration with the InternistOnCall and medical leads.
  • Collect document and submit referral and patient data for evaluation and research purposes to inform program improvements.
  • Build and foster external partnerships and community linkages to enhance service access and continuity.
  • Engage with PCPs through office visits educational sessions and enrollment activities to promote SCOPE services.
  • Identify barriers to system access contribute to the development of new care pathways and advocate for systemlevel improvements.
  • Participate in biweekly SCOPE Implementation Meetings Primary Care Project Meetings and other committees as required.

  • Ability to navigate complex health systems and work collaboratively with internal and external stakeholders.
  • Strong critical thinking decisionmaking and triaging skills under timesensitive conditions.
  • Highly organized with strong attention to detail in managing referrals documentation and program data.
  • Comfortable working independently while proactively seeking consultation for complex issues.
  • Ability to foster strong relationships with primary care hospital and community partners to promote integrated care.
  • Flexibility and adaptability to respond to evolving program needs and patient care challenges.

Qualifications :

Education:

  • Bachelor of Science in Nursing (BScN) required.
  • Current registration in good standing with the College of Nurses of Ontario (CNO).

Experience:

  • Minimum of 3 years of experience in acute care and/or primary/community care settings.
  • Experience working with older adult populations and patients with complex medical needs is an asset.
  • Demonstrated experience in interdisciplinary team collaboration and independent practice.

Skills:

  • Excellent interpersonal communication and relationshipbuilding skills.
  • Strong organizational skills with the ability to balance competing priorities.
  • Computer literacy required; experience with database management and electronic patient records preferred.
  • Demonstrated leadership ability and problemsolving skills.
  • Ability to travel within the community as required.


Additional Information :

Hours: MonFri 7.5 hour day shifts (Hours are subject to change based on operational requirements)

Salary: $46.004 to $59.637per hour
