The Discharge Coordinator is responsible for ensuring the timely discharge of medical and radiation oncology patients on the unit. This will be achieved through planning coordinating and facilitating the use of hospital resources to ensure efficiency in the discharge process and by evaluating discharge planning processes with the goal of ensuring appropriate lengths of stay on C2.
The Discharge Coordinator responsibilities include:
Maintaining the continuity of care when multiple services and care providers are involved on C2.
Attending daily interdisciplinary rounds team and engage in discussions and planning identify key areas to be coordinated
Assist in identifying discharge disposition for medical and radiation oncology patients to home or other care areas including but not limited to transitional care units rehabilitation programs etc.
Identifying services required from Ontario Health at Home for medical and radiation oncology patients being discharged home and initiate new referrals
Reviewing with Ontario Health at Home coordinator all patients with Ontario Health at Home referrals
Completing and submitting RM&R referrals and other applications in collaboration with the multidisciplinary team
Collaborating with the Team Leader and Social Worker with questions/troubleshooting regarding patient disposition planning
Initiating discharge planning as soon as possible after admission with the multidisciplinary team
Collaborating with other inpatient units and ambulatory areas to facilitate discharge plans for Medical and Radiation oncology patients.
Coordinating activities/appointments for patients with pending discharges
Anticipate discharge needs adjust plan of care with the interprofessional team to facilitate discharges
Facilitating all appropriate and documentation required for patient discharge
Developing an excellent working relationship with all areas/services that work closely with C2 staff and to be proactive to issues related to discharge planning
Completing documentation in a timely manner
Qualifications/Skills:
Registered Nurse (RN) with Current Ontario College of Nurses Certificate of Competence
Undergraduate degree preferred
Current B.C.L.S. Certificate
Minimum of 3 years recent experience working with Medical/Radiation Oncology patient population required
Completion of Oncology courses preferred
Certificate in Oncology Nursing preferred
Experience in discharge planning required
Demonstrated analytical and organizational skills required
Demonstrated process improvement and project coordination skills required
Demonstrated collaboration and team work skills required
Excellent communication/interpersonal skills required
Demonstrated satisfactory job performance and attendance
