Collaborate with clients families and service providers to develop personalised care plans that align with the clients goals preferences and available funding.
What youll be doing:
- Provide tailored episodic assistance to eligible clients including individuals from Aboriginal and Torres Strait Islander (ATSI) and Culturally and Linguistically Diverse (CALD) backgrounds to identify and overcome barriers to accessing services that meet their needs.
- Conduct individualised assessments of clients ensuring a comprehensive understanding of their needs preferences and challenges to create effective personcentred care plans.
- Identify and address gaps in access to services for SAH HACC PYP and CHSP target groups.
- Evaluate and assess the effectiveness of the ADS intake offering recommendations and assistance to ensure responsiveness of our actions
- Facilitate access to available services for target groups by providing initial direct support and linking clients to relevant services and programs.
- Actively promote and contribute to processes and systems that facilitate effective teamwork communication and collaboration across teams.
- Engage with colleagues and stakeholders by actively listening valuing diverse perspectives and involving others in decisionmaking and problemsolving.
- Take ownership of personal and professional development by actively seeking out learning opportunities training and feedback to enhance skills and knowledge.
- Stay informed of industry trends updates and best practices by reading relevant literature attending professional networks and engaging in external learning opportunities.
The skills and experience youll bring to the role:
- Demonstrated knowledge and understanding of the Aged Care Act and relevant regulations including compliance with the Aged Care Quality Standards and legislative requirements related to assessment processes.
- Highly developed skills and experience in providing assessments to clients.
- Ability to liaise and consult with medical allied health and other community professionals at all levels.
- Ability to work with and assist other staff to effectively support clients according to their individual needs and personal preferences
- Highly developed written and verbal communication skills with the ability to write reports and prepare correspondence.
- Ability to communicate effectively with and gain the cooperation of Council staff recipients and residents from a diverse range of age groups cultures and backgrounds.
- Ability to update and maintain administrative records policies and guidelines.
- Demonstrated ability to deal with difficult situations resolve intraorganisational problems and present a positive Council image.
Required Certifications & Qualifications:
- Degree or diploma in a relevant field (e.g. Aged Care Health Community Services) or lesser formal qualification with substantial experience.
- Current drivers licence.
