About the role:
You will manage a caseload of communitybased elderly individuals with Home Care Packages around the South West Sydney area of Sydney. You will assess their needs develop personalised care plans and manage budgets in accordance with program guidelines to ensure each customers individual assessed care needs are met. Youll also onboard new customers and proactively manage existing ones with a personcentred approach. Collaboration with our other internal Service Delivery teams will be crucial to delivering comprehensive care.
This opportunity is for an initial period of 12 months (with potential to extend or go permanent). You will have the flexibility to manage your own schedule each week while working autonomously from home and within the community. Starting asap you will regularly visit customers in their homes therefore you will ideally be located in or close enough to the southwest Sydney region.
