Community Care Tasmania is a not for profit focusing on providing compassionate care and support enabling our clients to thrive at home with dignity. We are seeking a dynamic and dedicated individual to become one of the team in our Launceston Office.

We are looking for a CHSP Coordinator to fill a 2 year fixed term position with the potential to become permanent.

Summary:

The purpose of this position is to assist in leading the delivery of the Commonwealth Home Support Program (CHSP) in line with identified clients needs and choices where practical as per the CHSP guidelines and requirements. This role will also be responsible for coordinating an effective service delivery through efficient rostering practices.

The CHSP & Home Modification Coordinator is accountable to ensure services are consistent with the relevant legislation quality framework and funding guidelines through clear communication system administration and linking individual work to funding outputs to ensure appropriate planning of services are consistent for Community Clients across Tasmania.

Our Launceston team are based in a brandnew state of the art building and have their very own clinical team (RN & EN) who are on hand for advice and support. The team provides statewide services.

Duties:

  • Manage the CHSP Dashboard case load and referrals through the My Aged Care Portal and accept in a timely manner in collaboration with the Intake Officer.
  • Undertake responsibility for all modifications including planning coordination implementation and administration.
  • Liaise with allied health professionals and qualified tradespeople to ensure the appropriate delivery of modifications meet clients needs are delivered within budget and meet requisite building codes.
  • Provide a coordinated service through ongoing assessment and response to the needs of clients from all backgrounds including those from a diverse cultural and linguistic background who have multiple needs (primarily through the Commonwealth Home Support Programme) including appropriate referral to other services.
  • Manage MAC portal including information and program availability
  • Manage CHSP reviews when required
  • Monitor and respond to all case notes and feedback from Support Workers.

About you:

We are looking for and organised and enthusiastic individual who wants to make a difference and holds the following qualifications and skills.

  • Cert IV or Diploma in Community Services/Case Management (desirable)
  • Minimum of 2 years of experience in case management of Home Care Packages or similar (desirable)
  • Police check (less than 3 months)
  • Current WWVP with NDIS endorsement
  • Drivers license
