Job Description
A growing organization is seeking a proactive and strategic Care Management Director to build and lead its care management department from the ground up. This leadership role involves designing the program structure developing policies and workflows hiring and training staff and ensuring all operations meet regulatory standards. The ideal candidate will have strong leadership and organizational skills experience in health or human services and a passion for building systems that deliver effective personcentered care.
Responsibilities Include:
Develop and implement the care management program structure workflows and policies.
Oversee the recruitment hiring training and supervision of care management staff.
Ensure compliance with all applicable regulatory standards policies and procedures.
Establish systems for client assessment care planning service coordination and followup.
Collaborate with internal teams providers and community resources to ensure comprehensive personcentered care.
Monitor program performance implement quality improvement initiatives and report key metrics to leadership.
Foster a culture of accountability excellence and compassion within the care management team.
Serve as the primary point of contact for regulatory agencies and oversight bodies related to care management services.
Ideal Qualifications:
Proven leadership experience in care management case management or health/human services program operations.
Strong knowledge of care coordination models healthcare regulations and best practices.
Excellent organizational problemsolving and teambuilding skills.
Ability to build systems and programs from the ground up including staffing workflows and compliance processes.
Effective communicator with the ability to build collaborative relationships internally and externally.
