We are seeking a compassionate organized and detailoriented Medical Office Assistant / Patient Care Navigator to join our healthcare team. This hybrid role plays a key part in both administrative operations and patient care coordination ensuring a seamless and positive experience for patients from their first contact through followup care.
POSITION HIGHLIGHTS:
Patient Navigation Duties:
- Helps guide patients through the healthcare system by assisting with appointments referrals and followups
- Assists patients with the patient assistance program application and ensuring accurate documents are provided
- Coordinate care plans between providers specialists and services
- Educate patients on procedures treatments and care instructions
- Support patients in understanding their insurance benefits and outofpocket responsibilities
- Identify and address patient barriers to care such as transportation or financial concerns
- Coordination with Optum Specialty Pharmacy Epclusa Mayvret and more as needed for best patient outcomes regarding grants to fund medication or patient assistance programs
- Provide emotional support and advocacy for patients throughout their care journey
Administrative Duties:
- Assists with sending and receiving medical records
- Schedule and confirm patient appointments using electronic medical records (EMR)
- Maintain and update patient records with accuracy and confidentiality
- Handle phone calls emails and other communications
- Process insurance forms and verify coverage
- Perform general clerical duties including scanning faxing and filing

- Able to work with basic computer skills. Must be able to work proficiently in EHR.
- Ability to network and coordinate efforts between treatment medical and other support systems for the patients.
- Must be able to physically perform the basic life operational functions of climbing stooping crouching reaching pushing pulling lifting standing walking fingering grasping talking and hearing.
- Must be able to perform sedentary work exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift objects.
- Must possess the visual acuity to perform extensive reading and determine the accuracy of work assigned.
