Job Description

InfuCare Rx is a leading home infusion provider accredited by URAC and ACHC offering a specialized approach to caring for patients in need of IV therapy and specialty pharmacy services in the homecare setting. Job Description:

As a Pharmacist youll be involved with multiple facets of the home infusion process as you interact with insurance companies staff members distributors Physicians/Nurses and patients to ensure accurate and safe quality service. You will use your experience as part of the Pharmacy Team to resolve problems when unforeseen roadblocks arise. Perform specific distributive and clinical pharmacy activities and related services as they pertain to providing safe and effective home infusion therapies.

Primary Duties:

The following duties are not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

  • Adheres to organization policies and procedures.
  • Coordinates and facilitates new referral/admissions through collaboration with nursing customer service and distribution.
  • Receives reviews and verifies all new patient orders and documents are the same on the order form.
  • Assists the physician and other healthcare professionals involved in planning and implementing patient care in the home to ensure safe and effective drug therapy.
  • Conducts initial and ongoing assessment of specific patient drug and disease information to identify real and potential drugrelated problems or needs.
  • Assists the physician and nursing staff in setting therapeutic objectives for drug therapy and establishing parameters for monitoring the safety and efficacy of drug therapy.
  • Monitors the outcomes from drug therapy by collecting evaluating and interpreting the results of laboratory data clinical data and other pertinent patient information.
  • Communicates the results of therapeutic drug monitoring activities to the physician nursing and other appropriate persons.
  • Programs infusion control devices as indicated.
  • Provides consultation and answers inquiries from patients families physicians and other healthcare professionals regarding drug and their use.
  • Will serve as designated person for all sterile compounding activities if applicable
  • Maintains thorough and appropriate documentation in the patient/client record of all clinical activities and communication with patients physicians and other healthcare professionals.
  • Participates in the preparation of progress reports for the referring physician on a regular periodic basis.
  • Supervises coordinates and/or participates in the preparation of sterile admixtures dispenses all pharmaceutical and related supplies and equipment in accordance with federal state and local laws and standards of practice which govern the profession of pharmacy.
  • Processes and checks all medications supplies and equipment prior to dispensing from the pharmacy.
  • Maintains pharmacy records in accordance with applicable laws and regulations.
  • Maintains clinical skills and knowledge of pharmacotherapy by reviewing the literature and attending inservices seminars and conferences.
  • Participates in oncall rotation if applicable.
  • Performs marketing support functions as requested.
  • Participates in orientation/training programs for new personnel and current staff.
  • Participates in departmental QA activities.
  • Attends designated department meetings as scheduled.
  • Performs other tasks or special projects as requested by management.

Confidentiality and Compliance:

  • Maintain confidentiality of patient and proprietary information at all times.
  • Observe legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary company information.
  • Understand and comply with all HIPPA requirements and regulations
  • Participate in the process for assessing managing and reporting medicationrelated incidents and equipment errors and/or failures. Accurately completes occurrence forms and forwards them to the appropriate governing bodies.
  • For all activities complies with accreditation legal regulatory and safety requirements.

Qualifications: (Education Certification/Licensureand Experience/Skills):

  • A Bachelor of Science in Pharmacy (RPh) or Doctor of Pharmacy (PharmD) from an accredited (Council on Pharmaceutical Education) college of Pharmacy.
  • Currently Licensed as a registered pharmacist in the state of employment.
  • Possess a minimum of two years recent experience in an acutecare setting or oneyear recent experience in home infusion therapy.
  • Possess experience and strong knowledge base in intravenous admixture services and intravenous therapy.
  • Must be capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians nurses and other healthcare professionals.
  • Can work effectively with others to provide safe and effective drug therapy.
  • Can communicate both verbally and written with patients clients management and other employees.
  • Can problem solve and supervise technical and other support staff.
  • Must be flexible with work schedule.
  • Possess good organizational and timemanagement skills.
  • Participation and membership in professional societies and organizations is desirable.
  • Dependable in attendance and job performance.


Physical Demands:

Required to stand walk sit talk and hear; required to use hands to operate vehicles and office equipment; must be able to occasionally lift and/or move up to 25 lbs; specific vision requirements for this job include: close distance and peripheral vision and the ability to adjust focus reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this position without compromising patient care.
