Job Description
Inhouse therapy team smaller daily caseloads with emphasis on optimizing time with patients! Chandler Therapy and Living Center provides short and longterm nursing and rehabilitation services to the senior population in our community. Speech Therapist Job Responsibilities: Develop and implement Speech Therapy plan of care for facility residents and shortterm patients. Assist as needed with patient screens for Speech Therapy. Communicate progress and any barriers toward SLP goals with interdisciplinary team members. Complete timely Speech therapy documentation in compliance with regulatory licensing facility and payor requirements. Assist with maintaining both the rehab gym equipment and treatment area in a safe manner using good infection prevention methods. Provide and document patient family and caregiver training regarding each speech therapy patient s specific plan of care and recommendations for follow up. Maintain productivity standards as expected by Director of Rehab or Administrator. Education Experience and Licensing Requirements: Current Oklahoma licensure as a Speech Therapist New grads welcome to apply! Work setting: Inperson Long term care Nursing home Outpatient Rehabilitation center Ability to commute/relocate: Chandler OK: Reliably commute License/Certification: Oklahoma Speech Therapy License (Required) Work Location: In person
