Project Healthy Grandparents (PHG) is dedicated to enhancing the wellbeing of families where grandparents are raising their grandchildren in parentabsent households. Since its inception in 1995 PHG has positively impacted over 3000 individuals by providing comprehensive services such as home visits parenting education legal referrals early intervention and transportation assistance.

The Lewis College of Nursing and Health Professions is seeking to hire a Social Worker II Sponsor Funded Professional to join the team at Project Healthy Grandparents.

WHAT WILL YOU DO:

As a Social Worker II Sponsored Funded Professional you will be an integral part of an interdisciplinary team focusing on direct assistance to grandparents raising grandchildren in parentabsent households. The primary service approach involves homebased case management with a specific emphasis on families with grandchildren aged birth to five years who may be at risk for developmental delays.

DUTIES AND RESPONSIBILITIES INCLUDE:

  • Conduct monthly case management home visits with families.
  • Utilize the Ages & Stages Questionnaire to screen grandchildren aged birth to 5 years for developmental delays.
  • Refer grandchildren with potential delays to the Neurodevelopmental Exposure Clinic at Emory University School of Medicine (ENEC) for a comprehensive evaluation.
  • Assist grandparents in completing the application process for the developmental evaluation at Emory University.
  • Accompany grandparents to ENEC for postevaluation meetings.
  • Provide case management services to grandparents based on clinicians recommendations.
  • Offer individual parenting education support services to families.
  • Recommend/provide developmentally appropriate toys books or educational resources to grandchildren.
  • Collaborate with grandparents and school personnel for schoolage grandchildrens Individual Education Plans (IEP).
  • Prepare monthly program service reports progress notes and maintain accurate documentation using computer database and/or spreadsheet programs.
  • Coordinate scheduling for service referrals and transportation for families as needed.
  • Interface with Project Healthy Grandparents interdisciplinary team and other community partners.
  • Attend departmental meetings trainings and required workshops.
  • Organize and facilitate monthly support group and parenting group meetings as needed.

SPONSOR FUNDED:

***This is a regular fulltime position which involves oncampus and online duties with job continuation dependent upon grant funding.

Bachelors degree and three years of related experience; orMasters degree and one year of related experience; or a combination of education and related experience.
