Key Responsibilities
-
Conduct skilled nursing assessments in the home and contribute to interdisciplinary care plans
-
Deliver and document care aligned with physician orders and patient needs
-
Administer medications perform ordered treatments and ensure a safe home environment
-
Educate participants and their families on disease management selfcare and preventative health
-
Coordinate care transitions between hospitals skilled nursing facilities and home
-
Track and manage visit schedules in coordination with the Home Health Manager
Qualifications
-
Graduate of an accredited School of Nursing
-
Active unrestricted RN license in California
-
1 year of nursing experience with elderly or frail populations (home health experience preferred)

Reliable transportation for local travel

Bachelors degree preferred
