Overview:The Housing navigator supportive services to individuals and families who are experiencing streetlevel homelessness. City Nets services are housingfocused; Housing Navigators work with clients on developing longterm housing plans. Housing Navigators connect homeless neighbors to appropriate supportive services including medical health treatment mental health treatment counseling and permanent housing.
Housing Navigators offer care and service coordination in partnership with other nonprofits County and City entities law enforcement behavioral health services and medical providers to ensure clients receive the help they need. These services are provided in collaboration with our partners with the intention of assisting clients with obtaining and maintaining permanent housing as quickly aspossible. City Nets programs follow Housing First and Harm Reduction service models. Expected Hours:This is a regular fulltime hourly position that works 5 days a week 8 hour day. Main Work Location:San Bernadino CA 91761 Please note:While this job posting is for an opening we currently have in the City mentioned above we are responsive to the needs of our clients and funders and therefore the person in this position may be transferred to neighboring locations as needed. Skills: - Conduct outreach to individuals and families experiencing homelessness or at risk of homelessness in various locations including shelters street outreach community centers and other public spaces.
- Build relationships with community organizations landlords and property managers to identify and secure housing opportunities for clients.
- Provide assessment and support to clients to identify their needs strengths and barriers to obtaining and maintaining stable housing.
- Assist clients in accessing and applying for affordable housing programs rental subsidies and other financial assistance as appropriate.
- Connect clients with social service agencies healthcare providers and other resources to address any physical or mental health issues financial concerns or other barriers to housing stability.
- Work collaboratively with other service providers that client is connected to ensure optimal service coordination.
- Facilitate and coordinate communication between clients and landlords property managers and other service providers to ensure a smooth transition into permanent housing.
- Develop and maintain client files and documentation according to agency and program guidelines including entry into HMIS and other databases.
- Participate in regular training and professional development opportunities to stay up to date on housing policies and best practices.
- Attend community meetings and events related to homelessness and housing to represent the agency and advocate for clients.
- Provide culturally appropriate health education information and outreach in communitybased settings such as homes clinics shelters local businesses and community centers for various Managed Care Plan (MCP) members.
Nice To Have - Personal lived experience of homelessness substance use disorders chronic health and mental health concerns domestic and intimate partner violence and/or human trafficking
- Experience gathering and maintaining confidential client records including intake paperwork case notes and client management system data entry
