Clinton Ave Suite 100 Rochester NY 14618 Float to the following locations as needed: 1. Medical Office Building: 1415 Portland Ave Suite 350 Rochester NY 14621 2. Linden Oaks: 30 Hagen Drive Suite 100 Rochester NY 14618 3. Greece Canal Landing 101 Canal Landing Blvd suite 8 Rochester NY 14626 EPIC experience including in basket work able to take feedback demonstrates flexibility medication prior auths.
