Program: Site Based Prevocational Services at LAICO
Position Title: Transition and Placement Coach
Responsibilities:
Prepares people referred by ACCESVR for competitive employment identifies job opportunities provides onsite follow up completes various marketing strategies to area employers assesses abilities based on the needs of the employers and makes sound job matches. Facilitates ACCESVR services including but not limited to PreETS services Work Experience Development Community Based Work Assessment and Project Search.
Essential Duties:
- Develop and coordinate all assessments and training opportunities that are requested by ACCESVR to assess and prepare students and adults for appropriate level of employment.
- Conduct program intakes for ACCESVR referrals and complete necessary documentation.
- Identify and act as a liaison with potential employers and market supported employment services. Assess work environment and job tasks to assure appropriate job match.
- Maintain familiarity with the ACCESVR contract and work together with agency and department staff to ensure that contract requirements are achieved by securing stable jobs in the community for the people referred for Job Development.
- Assist people supported in developing appropriate job behaviors work habits and appropriate relationships with employers supervisors and coworkers required to obtain and maintain positions in the community.
- Assist people supported with job interviews resumes applications and the job development process.
- Develop and implement Service Plans at Intake. Review and update Service Plans semiannually annually and complete amendments as requested. Make recommendations regarding employment opportunities.
- Serve as a positive role model for people supported. Interact with others in a way that will assist people in developing behaviors skills and attitudes necessary for successful community employment. Maintain neat appearance hygiene and grooming. Promote positive behaviors and interactions with all people supported families customers and all other support staff.
- Conduct a minimum of two case reviews per year for each persons Service Plan. Team meetings will be held on an as needed basis. Facilitate reviews in professional manner.
- Maintain records in an orderly manner including but not limited to service notes service plans and updating of information. All documentation is required to follow Medicaid OPWDD ACCESVR and Department of Labor Regulations.
- Participate in staff meetings agency trainings NY CASE trainings and inservice trainings.
- Initiate and maintain family contact and advocacy where indicated.
Qualifications:
Bachelors degree and oneyear experience in employment counseling and interviewing personnel administration or industrial relations or a masters degree in vocational rehabilitation or related areas such as personnel counseling and guidance and equivalent of oneyear experience working with the Developmentally Disabled population. Must meet LAIs Vehicle Operator Requirements. Demonstrates knowledge of local labor markets is strongly desired Committed to working with people with developmental disabilities and demonstrated ability to market their skills and abilities. Ability to complete an assignment without close supervision.

Commensurate with experience.

Monday Friday 8:00 AM 4:30 PM.
Require Meetings
Two staff meetings per month.
