Nasson Health Care
A Division of York County Community Action Corp.

Nasson Health Care is a Federally Qualified Health Center with clinical sites in Springvale and Biddeford Maine. We serve York County and neighboring communities through provision of fully integrated medical dental and behavioral health care.

Referral and Medical Record Specialist
40 hours/week; 52 weeks/year

The Referral and Medical Record Specialist works collaboratively with a team of health professionals to deliver comprehensive primary care services to patients employing the PatientCentered Medical Home model of care delivery. This dual position is responsible for managing all aspects of patient referrals for subspecialty care while providing critical clinical support duties. Additionally the Specialist plays a vital role in the maintenance and organization of the Health Centers hybrid medical record system. This includes the integration of paper documentation electronic health records and the utilization of health information exchange databases. By effectively evaluating health information the Specialist ensures accurate and uptodate electronic medical records facilitating seamless coordination of care for all patients.

Duties:

  • Retrieves Medical records as requested by healthcare providers patients and authorized personnel.
  • Responds to requests and prepares for the submission of medical records requests from external entities such as attorneys insurance companies and government agencies.
  • Monitor referral statuses follow up with specialists and document all referral activities.
  • Maintain a referral tracking system and generate performance measures.
  • Actively participate in quality improvement efforts at the department health center and agency levels.
  • Represent YCCAC/Nasson professionally in interactions with patients colleagues and the community.
  • Comply with all applicable federal state local and YCCAC/Nasson regulations rules protocols and procedures related to primary care services workplace safety public health and confidentiality.

Qualifications:

  • Must possess an associate degree in medical assisting medical records or health care related field preferred.
  • Minimum of three years experience working in a health care setting.
  • Familiarity with medical terminology and diagnostic and procedure codes.
  • Demonstrates proficiency in the use of standard office software and an electronic health and practice management systems.
  • Actively practices strong written oral communication and organizational skills to attend multiple tasks prioritize time sensitive tasks and ensure compliance with established deadlines and protocols.

Must show proof of immunity or have been immunized for MMR Hep B Influenza (required annually) and the COVID primary series.
