The Community Subacute and Transitional Care Center is a 106bed skilled nursing facility (SNF) comprised of two unique and distinct units: Subacute and Transitional levels of care. This facility provides inpatient care for chronic subacute conditions and other complex illnesses in addition to the extended skilled nursing level of care. As a distinct part SNF of our Level I Trauma Center Community Regional you will be integral to the continuity of care across our health system.
Responsibilities
The role of the clinical nurse is to deliver patient care through assessment planning implementation and evaluation of patient needs with the highest professional standard while promoting nursing excellence through continued education and research based on the Title 22 and Title 16 the ANA Scope & Standards of Practice the Code of Ethics.
