Job Description

Located in the City of Frederick TheTransitional Age Youth (TAY) Programis an innovative timelimited 8bed residential and psychiatric rehabilitation program that is specifically tailored to meet the special needs of young adults between the ages of 1823. This unique program transforms the lives of its young residents by providing a structured homelike environment in which young adults are supported in developing daily living skills as they successfully transition to adulthood and life in the community. The program engages young adults in meaningful and productive activities such as school volunteer work and/or paid employment as well as other goaloriented activities that promote independent living and healthy life choices.

What to expect.

  • Engage young people through relationship development personcentered planning and a focus on their futures.
  • Tailor services and supports to be accessible coordinated developmentallyappropriate and build on strengths to enable the young people to pursue their goals across all transition domains.
  • Acknowledge and develop personal choice and social responsibility with young people.
  • Ensure a safetynet of support by involving a young persons parents family members and other informal and formal key players.
  • Enhance young persons competencies to assist them in achieving greater selfsufficiency and confidence.
  • Maintain an outcome focus in the TIP system at the young person program and community levels.
  • Involve young people parents and other community partners in the TIP system at the practice program and community levels.
  • Locate and arrange for participation in appropriate services within the agency and in the community for TAY.
  • Work collaboratively with families vendors referral sources and multidisciplinary staff to provide quality services that meet the needs of families with special needs.
  • Coordinate/implement the assessment and program planning process.
  • Serve as a liaison with referral sources school personnel and families including initiating and/or attending any outside case conferences team meetings or phone contacts necessary to facilitate a continuum of care for each youth
  • Ensure completion of written documentation according to grant and Way Station Inc. program standards including admission treatment and discharge paperwork.
  • Implement & monitor quality improvement measures.
  • Utilize creative strategies to engage difficult youth.
  • Assist with orienting youth new staff members volunteers students families and others as needed to the program.

What we need from you.

  • Your high school diploma or a GED. Bachelors degree in a related field is preferred.
  • Professional Experience: Two years positively referenced relevant work experience with TAY in an educational or social service setting and/or general vocational knowledge.
  • The willingness to obtain additional certifications and trainings as required.
  • A drivers license with 3points or less and access to an insured vehicle.
  • Must have availability to workdays nights weekends and holidays depending upon need of the program.
  • Computer Skills: Familiar with Microsoft Office proficient with keyboard to input information into electronic health record
