Job Description

Description

The Registered Nurse assesses plans implements and evaluates the nursing care provided to agroup of patients. This nurse leads the nursing team members in the care as directed by theattending physician and in collaboration with other health care providers in accordance with thephilosophy and policies of ProMedica Acute Care Hospitals. Actively participates in the creation ofan environment that fosters patient family physician and employee satisfaction. Ensuresdecisions made are based on patient and family centered care philosophy utilizing evidencedbased practices and focused on safety customer satisfaction and quality outcomes.

Scope of Service: The Registered Nurse provides nursing care to individuals and groups whichrequire specialized knowledge judgment and skill derived from the principles of biologicalphysical behavioral social spiritual/cultural and nursing sciences. The Registered Nursefunctions within the full scope of nursing practice as noted by the Board of Nursing of theirdesignated state for which they are practicing in addition to compliance with any of the hospitalsaccrediting bodies.

ACCOUNTABILITIES

1. Patient & Family Centered Care

a. Recognize the patient or designee as the source of control and full partner in providingcompassionate and coordinated care based on respect for patient preference valuesand needs.

b. Elicit and communicate patient values preferences and expressed needs.

c. Provide patientcentered care with sensitivity and respect for the diversity of humanexperience in collaboration with the interdisciplinary team. Engage patients ordesignated surrogates in active partnerships that promote health safety and wellbeing and selfcare management.

d. Addresses patient/family education needs considering patient level of comprehensionstyle of learning from the point of entry through discharge.

e. Navigates and collaborates with patient family and healthcare team regarding the careprovided and needed to ensure appropriate transition along the continuum.

f. Evaluates patients response and intervenes to ensure optimal patient outcomes.

2. Evidenced Based Practice

a. Integrate best current evidence with clinical expertise and patient/family preferencesand values for delivery of optimal health care.

b. Base individualized care plan on patient values clinical expertise and evidence.

3. Nursing Practice

a. Assesses formulates nursing diagnosis plans and implements and continuallyevaluates effectiveness of the individualized written nursing care plan and delivery ofnursing care for each patient assigned.

b. Analyzes and prioritizes problems and makes sound judgments based on logic and/orcritical thinking skills.

4. Safety and Quality Improvement

a. Improve the safety and quality of healthcare systems by minimizing the risk of harm topatients and the healthcare team.

b. Administers medications and executes regimens as authorized by the state licensure.

c. Incorporates quality measures and data to understand individual and systemperformance.

d. Applies use of technology and standardized practices that support safety and quality.

e. Identifies strategies to reduce risk of harm to self or others.

f. Participates in unitbased quality improvement evidencebased practice projects orresearch.

g. Promotes and incorporates error prevention techniques.

h. Report safety concerns and events including near misses.

5. Informatics & Technology

a. Promote and incorporate use of technology to communicate manage informationmitigate errors and support decision making.

b. Navigates plans patient care and documents ongoing status interventions patientresponses and outcomes in accurate timely manner in the electronic health record.

c. Responds appropriately to clinical decisionmaking supports and alerts.

d. Advocates protecting the integrity of the healthcare record and the privacy of eachpatient.

6. Communication Teamwork & Collaboration

a. Interacts effectively with patient families and with healthcare team to foster opencommunication mutual respect and shared decisionmaking.

b. Functions as the coordinator of the patients plan of care through interaction andcommunication with healthcare team.

c. Acts with integrity consistency and respect for diversity.

d. Functions within scope of practice.

e. Delegates to other team members as appropriate.

f. Escalates barriers safety and clinical concerns utilizing appropriate chain of command.

7. Leadership & Professionalism

a. Influences the behavior of individuals or groups within the environment to facilitategoal establishment and achievement. Accountable for the delivery of nursing careconsistent with the core values mission philosophy and policies of ProMedica.

b. Understands and maintains a current working knowledge of scope of practice asoutlined by state Board of Nursing.

c. Integrates leadership skills to promote systems thinking communication andfacilitating change.

d. Participates in healthcare teams to improve patient care the work environment andpatient and staff satisfaction.

e. Accountable to own nursing practice by following nursing process. Assigns directsdelegates and supervises ancillary personnel and support staff in carrying outparticular roles/functions aimed at achieving patient care goals.

8. SystemsBased Practice

a. Participates in practices endorsed at the healthcare system level.

b. Plans organizes and delivers patient care in the context of the work unit.

c. Rounds with providers.

d. Practices bedside report/handoffs.

e. Ensures meaningful hourly rounding occurs.

f. Escalates concerns utilizing appropriate chain of command to maintain safety qualityand patient satisfaction.

9. Other duties as assigned.



Requirements

REQUIRED QUALIFICATIONS

Education: The RN in Ohio must complete twentyfour contact hours of continuing educationduring his/her licensure period. At least 1 hour of contact hours must be related to Chapter 4723of the Ohio nurse practice code and rules.

Skills:

Ability to respond effectively and efficiently to critical situations involving patients personneland visitors in a professional manner.

Performs basic nursing skills competently.

Ability to evaluate own daily practice for errors and pursue corrective action.

Responsible for continued professional growth and development and competency.

Navigate and retrieve information from the computer and healthcare record.

WORKING CONDITIONS

Personal Protective Equipment:

Demonstrates knowledge and use of standard precautions and PPE.

Demonstrates knowledge and use of safe patient handling equipment.

Physical Demands:

Must be able to tolerate exposure to dust fumes chemicals temperature changes andexposure to blood borne pathogens and bodily fluids.

Must be able to attain all health requirements as identified by the organization (i.e. PPDrespirator training vaccines as appropriate).

Must be able to move about hospital and between workstations and prolonged periods ofstanding. **

Must be able to frequently move lift or carry light to heavy patients or equipment. **

Must be able to lift medium materials nonmaterial handling; continuous standing walkingbending stooping and reaching; frequent pushing pulling and squatting.

Occasional sitting stairs and kneeling. **
