Job Description:
We are seeking a skilled Clinical Pharmacist with experience in senior care and dementia patients to join our team on a contracted per diem basis. The ideal candidate will collaborate closely with healthcare providers to offer expert consultation on highrisk medications and deprescribing strategies.
Key Responsibilities:
Collaborate with healthcare providers to assess medication regimens and identify opportunities for optimization particularly in seniors and dementia patients.
Provide recommendations for deprescribing medications that may pose risks or have limited benefits while ensuring patient safety and wellbeing.
Develop and implement individualized treatment plans based on patient needs medical history and best practices.
Conduct comprehensive medication reviews and reconcile medication discrepancies.
Educate patients caregivers and healthcare providers on medicationrelated issues including appropriate use adverse effects and alternatives.
Document interventions and communicate findings effectively to the healthcare team.
Requirements:
Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
Active pharmacist licensure in CA.
Minimum of 2 years of experience in clinical pharmacy practice with a focus on senior care and dementia patients preferred.
Strong knowledge of geriatric pharmacotherapy including principles of deprescribing and medication management in patients with cognitive impairment.
Ability to work independently and collaboratively within a multidisciplinary team.
Excellent communication and interpersonal skills with a demonstrated ability to interact effectively with healthcare providers patients and caregivers.
Detailoriented with strong analytical and problemsolving abilities.
