Together we bring exceptional home health hospice and personal care services to more than 3000 Missourians in our local communities every day.
As a home care Home Health Physical Therapist in Southwestern Missouri you will be an integral member of a multidisciplinary health care team that provides rehabilitative care and skilled nursing to clients affording them the opportunity to receive the medical care to remain at home.
This is a Per Diem position.

Our Physical Therapists deliver exceptional care to every client.
- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements such as the mobility/range of joints transfer status stability patterns and appearance of ambulation strength and endurance of muscles balance testing and safety assessments.
- Develop and implement appropriate individualized care plans including manual therapeutic exercises gait training balance and other interventions.
- Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidencebased dressings and topical agents
- Continually assess and revise the Physical Therapy PT care plan
- Educate and instruct clients family members or other client representatives in rehabilitative care and activities necessary to promote the clients health safety and independent living.
- Accurately document observations interventions and evaluations pertaining to client care management
Qualifications for an Integrity Physical Therapist:
- A currentPhysical Therapist PT license in the state of Missouri.
- Previous home health care experience welcomed but not .We will train you!
- Solid computer skills; prior experience with electronic medical records (EMR) preferred
- Ability to travel locally to cases as assigned
