The LPN Nurse Navigator position assists providers during forensic medical exams conducted at the Child Advocacy Center while assessing and caring for alleged victims of child maltreatment including physical and sexual abuse. The LPN Nurse Navigator will obtain and prepare lab specimens and convey exam results to multidisciplinary team partners. This position also assists medical providers in the Division of Child Protection and Advocacy by monitoring for inpatient consult requests coordinating care at the CAC after discharge and monitoring results received with notification to the ordering provider. Work is performed in a multidisciplinary manner working closely with Child Advocacy Center and Division of Child Protection and Advocacy staff as well as investigative agencies such as law enforcement and child protective services.Job Duties:
The position is fulltime 40 hours weekly. Hours are typically Monday through Friday 8:00 a.m. 4:30 p.m. At least two (2) years of LPN work experience is required.
Licensed Practical Nurse performing duties as a patient navigator guiding incoming calls and facilitating entrance of new patients into the program. Provides patient education promotes an understanding of the condition and provides ongoing continuity of care for patients. The Nurse Navigator functioning in the spirit of teamwork is accountable for the delivery of care utilizing skills learned in both the clinic and didactic arena. Assists other team members on the unit. The nurse navigator role provides clinical support to the provider and patients to assist in the diagnosis and treatment of conditions. Utilizes clinical knowledge to determine actions necessary to facilitate patient introduction to the program.
