REPORTING RELATIONSHIPS/SUPERVISORY RESPONSIBILITIES

This position reports to the Director Community Health & Impact and has one direct report the Coordinator Community Health & Engagement.

POSITION SUMMARY

The Manager Community Health Initiatives leads the design implementation and continuous improvement of initiatives that integrate clinical care and community health to improve care access and health in the communities we serve.

The manager partners with clinical service line and hospital teams philanthropy and grants marketingcommunications and other stakeholders across the organization. The position also builds relationships and works closely with community organizations and stakeholders.

ACCOUNTABILITIES

  1. Partner with clinical service line and hospital teams to design and implement initiatives that advance care access and community health improvement. Includes health literacy and community outreach evidencebased health promotion and disease prevention health related social needs patient and community feedback and engagement community forums and health & wellness events early career exposure and pathway programs and other initiatives.
  2. Develop and sustain stakeholder relationships and community partnerships that improve care access and community health. Foster community involvement in design implementation and improvement of initiatives to ensure services are culturally competent and responsive to the needs of different communities and populations.
  3. Partner with the community health & impact team to ensure community health initiatives align with community engagement & impact community benefit and community health needs assessment and improvement plan processes plans and actions.
  4. Develop performance measures and improvement processes and conduct datadriven evaluations to measure effectiveness of initiatives. Analyze clinical community and public health and community impact data to guide strategic decisionmaking.
  5. Ensure compliance with funding requirements grant agreements and organizational policies while reporting on key performance indicators for initiatives such as community health programs workforce training and health access interventions.
  6. Provide leadership to community health & impact staff and manage team resources and budgets effectively.
  7. Other duties as assigned.

REQUIRED QUALIFICATIONS

Education: Bachelors Degree in healthcare administration public health social work a related field or an equivalent combination of experiences.

Skills:

  • Works collaboratively with diverse stakeholders to solve problems resolve conflicts and make effective decisions.
  • Builds interpersonal trust and credibility with diverse stakeholders to develop implement effective initiatives and processes.
  • Communicates effectively by developing and delivering multimode communications to different audiences.
  • Strong ability to plan and coordinate complex projects manage timelines and lead teams to achieve project results.
  • Collects evaluates and understands data to measure and share outcomes.
  • Secures and manages grants and funding from public and private sources.
  • Provide leadership mentorship and professional development opportunities to cultivate a culture of collaboration innovation and accountability.

WORKING CONDITIONS

Personal Protective Equipment: According to facility requirements as determined by the location.

Physical Demands: Walking climbing steps lifting bending. Travel to various office and community work locations for meetings and events. Flexibility in work hours may be required to accommodate various stakeholders.
