Home Health Clinical Manager (RN)

Key Responsibilities

• Lead Clinical Teams: Supervise and support nurses therapists and home health aidesincluding hiring onboarding scheduling mentoring and performance evaluations.

• Coordinate Patient Care: Oversee individualized care plans in collaboration with physicians and allied health professionals to ensure timely and effective interventions.

• Ensure Compliance: Maintain strict adherence to federal state and local healthcare regulations including Medicare/Medicaid guidelines and Joint Commission standards.

• Drive Quality Assurance: Conduct audits track clinical outcomes and spearhead continuous improvement initiatives for optimal patient results.

• Advocate for Patients: Serve as a liaison to address concerns resolve issues promptly and enhance overall patient and family satisfaction.

• Manage Documentation: Review and approve clinical notes and assessments for accuracy completeness and regulatory alignment.

Required Qualifications

• Education: Bachelor of Science in Nursing (BSN) required; Masters in Nursing or Healthcare Administration preferred.

• Licensure: Active RN license in the state of Arizona.

• Experience: 23 years in home health or a related field with prior supervisory or management experience.

• Certifications: BLS certification required; Home Health Care Certification preferred.

• Technical Skills: Proficient in Electronic Health Records (EHR) systems and clinical software.

• Soft Skills: Strong leadership excellent communication decisionmaking and teambuilding abilities.

• Regulatory Knowledge: Deep understanding of Medicare/Medicaid reimbursement and home health compliance.
