Job Description
Hospice Case Manager (RN)
Were hiring a compassionate Hospice RN Case Manager for a wellmanaged stable hospice organization serving Washington and the Pittsburgh area since 2004. Youll provide skilled care focused on pain management wound care and patient comfort while guiding families through hospice admission and education. This role includes close coordination with the care team and participation in the oncall rotation to support urgent needs and endoflife transitions.

Responsibilities:
- Monday to Friday: 8:30 AM to 5:30 PM
- New Grads Welcome
- Washington PA Coverage Area
- Average Case Load of About 1621 Patients
- Average 46 Patients Per Day
- Limited OnCall Schedule
Requirements:
- RN Nursing License in the State of Pennsylvania
- Valid Drivers License with Acceptable Driving Record
- At Least One Year of Nursing Experience Preferably in Hospice or Homecare.
- Nurses with Oncology Med Surg and Oncology Experience are Also Urged to Apply.
