Job Role:

We are seeking a compassionate and motivated Support Worker to join our team specifically focusing on supporting individuals through Transitions. This role will involve helping individuals as they move through key changes in their lives home to independent living or supporting people with disabilities to adjust to new routines.

Key Responsibilities:

  • Provide personcentred support to individuals navigating life transitions.
  • Assist with developing and implementing tailored support plans to address unique needs.
  • Offer practical help and emotional support to enhance independence and wellbeing.
  • Build strong trusting relationships with individuals their families and other professionals.
  • Assist with daily living tasks including budgeting meal preparation and personal care as needed.
  • Promote inclusion advocacy and participation in the community.
  • Support individuals to develop skills that increase their confidence and selfsufficiency.
  • Encourage individuals to express their preferences values and needs.
