Job Description:

As a unionized role members of SEIU Local 1 Homestead will be reviewed ahead of any nonunion or external candidates. All union applications must be processed before considering other applicants.

The East Toronto HART Hub provides evidencebased healthcare treatments social services and housing support for individuals struggling with addictions mental health housing and other related issues. Clients will have lowbarrier access to primary care mental health and addictions treatment with direct referral to housing and other support services. The Hub is an initiative of East Toronto Health Partners Ontario Health Team (ETHP OHT). It is led by South Riverdale Community Health Centre and St. Michaels Homes with support from 2Spirited People of the 1st Nations Anishnawbe Health Toronto The Salvation Army and other community partners.

The Homestead Alice M. Walter House Scarborough Satellite Programs provides a variety of addictionspecific services to clients residing in Scarborough. The Addiction Case Management position entails working in conjunction with the Womens Supportive Housing Program as well as providing addiction case management support to the community

RESPONSIBILITIES:

  • Outreach and Engagement with clients
  • Pre and posttreatment support
  • Assessment and Referrals
  • Inform clients and the community (landlords property management companies other agencies etc.) about the Womens Supportive Housing Program
  • Assist admitted Womens Supportive Housing clients in locating suitable accommodations / rental units
  • Accompany clients to view rental units negotiate rental rates assist with paperwork to secure units
  • Individual Counselling
  • Development of case plans based on clientcentred recovery goals
  • Networking with community agencies
  • Crisis/Conflict Intervention
  • Client documentation
  • Community Followup
  • Facilitate Community Treatment groups (on an as needed basis for coverage)
  • Attend team meetings as required
  • Team Consultation and DecisionMaking
  • Report to Program Coordinator of Addiction Outreach Programs

ESSENTIAL SKILLS AND QUALIFICATIONS:

  • Undergraduate degree or diploma from a recognized institution (in one of the human services professions) including qualification to enter a Provincially Regulated College
  • Minimum 3 years experience working in the addiction/mental health sector
  • CACCF/ICADC Certification an asset
  • Understanding of the continuum of substance use and concurrent disorders
  • Knowledge and experience in women centered problematic substance use counselling interventions / treatment models
  • Knowledge and experience of concurrent disorders
  • Understanding of trauma in relation to substance use
  • Knowledge of culturally competent practices
  • Maintain client records / files according to agency standards
  • The ability to work individually and as part of a team
  • Demonstrated ability to meet deadlines
  • The ability to work collaboratively with community partners
  • Knowledge and experience with data management systems particularly EHMware

Compensation:

The target hiring range for this position is $22.97 to $22.97 with a maximum of $22.97.

Placement in the salary range will be based on factors such as market conditions internal equity and candidate experience skills and qualifications relevant to the role.
