The Nurse Liaison provides clinicallybased case coordination of efforts to support the delivery of effective and efficient home health and hospice care. Has the overall accountability for certified home health and hospice care coordination and discharge planning for patients referred to them. Collaborates with other members of the health care team to identify appropriate utilization of resources and to assure maximum reimbursement. With the patient family and health care team creates a discharge plan appropriate to the patients needs and resources.GENERAL DESCRIPTION: The staff R.N. under the supervision on the Nurse Director or designee is responsible for the nursing care of assigned psychiatric patients located in the Emergency Care Center (ECC). Included in such responsibility is the goal of facilitating ECC psychiatric patients admission process to the inpatient psychiatric units or the transfer of patients to outside facilities. This nurse will work closely with the ECC staff Psychiatric Emergency Services (PES) and the inpatient psychiatric units. S/he will maintain competencies in both the ECC and Inpatient psychiatric units and will work in the ECCs on both campuses (Southbridge and Webster) and float to the inpatient unit on 2 South based on needs of the departments as determined by the ECC charge nurse and/or ECC leadership representative.
WORK EXPERIENCE REQUIREMENTS: A minimum of 2 years emergency nursing or psychiatric nursing is preferred.
EDUCATION REQUIREMENTS: Graduation from an accredited school of nursing with a current license by the Massachusetts Board of Nursing. American Heart Association (AHA) Basic Life Support (BLS) certification for health care provider required upon hire or during orientation. Advanced Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certification is required within 6 months of hiring.
