office. The Lung Health Foundation is based in Toronto.
Salary range:$38.00 $45.00 / hour (Contract)

POSITION OVERVIEW
The Certified Respiratory Educator (CRE) supports the work of the LHF in delivering programs and services that improve the health of people living with lung disease as well as in promoting initiatives that help prevent lung disease in the broader community. The CRE plays a significant role in bringing clinical experience and expertise to deliver on the LHFs mission. Specifically this role will assist in delivering the Lung Health Coach program an evidencebased personcentered COPD selfmanagement and education program cofacilitate the lung health support group meetings and review clinical content including tools education and messages.
Given the clinical nature of LHFs programs and services this position requires a licensed provider with or willing to get a CRE designation.
Its an exciting time at the LHF where the team of Lung Health Coaches together with the Community Programs and Partnerships team is creating an inclusive proactive approach to welcome people in and provide them with the programs resources and linkages that they need to be supported along their lung health journey.
RESPONSIBILITIES
Clinical and Evidencebased champion
- Source of clinical knowledge and evidencebased information to inform the work of LHF (e.g. includes staying up to date on evidence and emerging issues)
- Maintains relevant knowledge and skills related to the role of the Lung Health Coach (CRE) including evidence motivational interviewing health promotion social prescribing persuasive communication conflict resolution
Knowledge Mobilization
- Responsibility to support knowledge mobilization efforts such as assisting with needs assessments and environmental scans to support service mapping and patient navigation
- Reviewing curriculum and tools to support behaviour change efforts at the LHF
Personalized support visits and engagement efforts
- Responsibility for the personalized support visits and inquiries to provide a clinical and/or education service for people with lung disease and their caregivers by determining their needs and providing the appropriate assistance regarding self management education and health system navigation
- Responds (as needed) to calls chats and emails about lung health providing clinical and/or education service for people with lung disease caregivers and/or providers
- Contributes to the personalized lung health service model including:
- Input into the service model and annual business planning
- Input and tracking in Salesforce to ensure appropriate data is being captured aligned to the evaluation framework at LHF and for reporting on reach uptake and impact
- Provides support in building a patient network across the province and nationally.

Patient Lung Health Support Groups
- Maintains a small case load of clients within the My Lung Health Coach program
- Promotes and cofacilitates the LHFs virtual support group
Patient Submissions:
- Reviews drug submissions to Ontarios Committee to Evaluate Drugs on behalf of Ontario patients and caregivers (as needed)
- Supports the data collection process through online surveys and telephone interviews (as needed)

QUALIFICATIONS
- Certified Respiratory Educator preferred or willing to take RespTrec training in asthma COPD and chronic disease education
- Must be a regulated healthcare professional in good standing;
- Bilingual in English and French (written and verbal) would be an asset
- Knowledge of Canadian Thoracic Society Respiratory and related guidelines based on our 5 strategic goals.
- Demonstrated knowledge in the concepts of health promotion patient education research secondary prevention and chronic disease management
