HealthPartners North Suburban Family Physicians has an exciting opportunity for a patientfocused Advanced Practice Provider to join our team and make a difference for our patients and our community. This Advanced Practice Provider will support clinic visits for an employer based clinic in Buffalo MN.

Wright County is an employerbased clinic supporting the healthcare needs of the employees and dependents of Wright County. In this role the Advanced Practice Provider will operate independently and have responsibility to check in room treat and dispense generic medications. Additional responsibilities include obtaining health history examining patients ordering appropriate lab and xray exams communicating with patients regarding treatment and follow up and interacting professionally with other staff. Care provided emphasizes health promotion and disease prevention diagnosis and evidencebased management of acute and chronic diseases and occupational health support.

Service excellence is to be centered on patient care and patient relationships and is the responsibility of all employees. Teamwork is the norm and all employees will be held accountable to work as effective team members. Assume delegated medical activities from supervising physician. This may include physical examination history taking developing comprehensive medical status reports formulating a tentative medical diagnosis and initiating therapy under medical supervision.

Provides a source of entry for the patient into the healthcare system through identification and management of healthcare of individuals and families (consistent with description of practice of certifying specialty). Care provided emphasizes health promotion and disease prevention as well as diagnosis and management of acute and chronic diseases. The Physician Assistant works autonomously as well as in collaboration with Physicians and other healthcare providers according to North Suburban Family Physicians and ICSI Guidelines. Patients are encouraged to be responsible for their own health behavior through education support and involvement in their plan of care.

ACCOUNTABILITIES:

Assesses the health status of individuals and families as appropriate

A complete health history is obtained and documented.

Patients perception of problem is elicited.

A complete accurate physical examination is performed and documented.

Appropriate screening and diagnostic tests are ordered and evaluated.

A problem list is developed.

Wellness and risk factors are identified.

Identify variations from norms and diagnose common health problems and diseases.


II. Develops and implements plan of care

Sets goals and plans mutually acceptable to patient and provider.

Prescribes a variety of medical treatment to meet wellness and illness needs includes but not limited to medication nutrition exercise and traditional nursing treatments.

Manages wellness and illness needs according to NSFP and ICSI Guidelines.

Teaches and supports positive selfcare activities including problem solving and selection of treatment alternatives.

Provides health counseling and emotional support throughout care.

Assists patient with health maintenance activities.

Chooses interventions which will promote the patients participation in his/her own care to the greatest possible extent.

Evaluates the process and outcome of a management plan and modifies the plan as appropriate.

Treats common childhood diseases.

Assists in the followup of geriatric disorders referred by physicians.

Prescribes medications for symptoms and temporary pain relief (if eligible).

Performs routine therapeutic procedures (i.e. injections debridement foreign body removal from skin surfaces removal of sutures casting or splinting of sprains I & D of superficial skin infections) as identified in the supervisory agreement referenced above.

Performs other activities identified as assigned.


III. Collaborates and coordinates with other health professionals as appropriate

Discriminates between situations requiring independent management collaborative management with physician and immediate or further referral.

Collaborates with physician when unusual incidents or changes in patients condition occur.

Coordinates care involving other healthcare providers.

Makes appropriate referrals.

Functions as a mentor resource and educator for other staff members.

Assists the physician in instituting emergency measures and treatment.


IV. Maintains and improves professional practice

Makes recommendations for improving care.

Maintains professional competency by remaining current in continuing education activities.

Arranges for periodic meetings with site chief.

Maintains current licensure and certification.

Incorporates current research into practice to improve practice.

Participates in peer review activities.

Receives yearly performance evaluation.

Practices in accordance with national standards of practice.

Appropriate documentation of care is done according to NSFP standards.

Participates in appropriate task forces for committees.


REQUIRED QUALIFICATIONS:

REQUIRED FOR PHYSICIAN ASSISTANT:

Graduate of an accredited formal Physician Assistant training program.

Current Minnesota PA license.

Current NCCPA certification.

Active member in professional PA association preferred.

Approved or eligible for approval by the Minnesota Board of Medical practice for prescription writing privileges.

REQUIRED FOR NURSE PRACTITIONER:

Graduate of an accredited school of nursing and formal nurse practitioner program. Masters required for candidates graduating after 1992.

Current RN License

Certified through the ANA national exam.

Current Minnesota APRN license.

Approved or eligible for approval by the Minnesota Board of Nursing for prescription writing privileges.

Nurse Practitioners beginning practice after July 1 2014 must practice for at least 2080 hours within the context of a collaborative management setting where APRNs and Physicians work together in a hospital or integrated clinical setting.

Current CPR certification for health professionals through the American Heart Association or Red Cross required and maintained. Online CPR certification is not acceptable.

MN Board of Nursing sets the definition for nursing practice and licenses APRN practice.

National Certification Organization ex: ANCC which certifies specialty practice.


PHYSICAL REQUIREMENTS:

Physician Assistants must possess body mobility coordinated dexterity of hands ears and eyes. Must possess sensory acuity and verbal ability. Lifting of infants/children and/or adults may be required. No lifting requirements are identified but the potential for lifting exists. Performing physical examinations communication with parents/patients and implementing medical/nursing treatments requires adequate vision hearing and tactile motor skills. Must be able to speak read and write English proficiently.
