Job Description
The LPN at the request of the Intake Department provides nursing services to patients as ordered by the physician. The OnCall Hospice LPN s primary responsibility is to respond to hospice patients and caregivers needs at the patients home

Hospice care may take place in a hospice institution or the patients home. Each hospice patient is cared for by a team of professionals who provide medical care pain management and emotional and spiritual support tailored to the patients wishes.

Primary Job Responsibilities
- The OnCall Hospice LPN s primary responsibility is to respond to hospice patients and caregivers needs at the patients home
- Provides patient revisits and pronounces
- Provides skilled nursing care focusing on pain and symptom management.
- Provides interim guidance to patients and caregivers regarding diagnosis medications and treatments to provide continuity of care and support for patients and caregivers.
- Works closely with the Interdisciplinary Team
- Documents with OASIS in a timely manner
Requirements
- Graduate of an accredited LPN program
- Current LPN license (PA)
- Minimum of one year experience in a hospice homecare setting
- Preference for knowledge of OASIS
- Maintains clinical and professional competence
- CPR certified
- Drivers License and Car Insurance in good standing
- Reliable vehicle
- Possesses the ability to follow oral and written instructions
- Possesses the ability to communicate effectively with all members of the care team
- Possesses good organizational skills.
- Proof of COVID19 Vaccination or an approved Medical or Religious exemption.
- Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption
