About Outpatient Sites

Corewell Health operates over 300 outpatient clinics across Michigan bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area.

Scope of work

Responsible for providing quality education followup and emotional support to the population and their families. Case manages the population throughout the inpatient and outpatient settings and measures outcomes to maximize continuity of care between the settings. Manages projects and programs related to clinical management performing quality improvement activities to measure program processes and outcomes.

Essential Functions

  • Assesses the educational needs of clients/others. Develops and implements appropriate educational curricula/teaching strategies or makes appropriate referrals. Provides ongoing review and development of educational materials related to clinical management.
  • Case manages the population both in the inpatient and/or outpatient settings. Develops and manages clinical paths as appropriate and measures outcomes of care to maximize continuity of care and appropriate resource utilization and length of stay. Coordinates multispecialty team clinics.
  • Provides direction to specified projects/programs pertaining to the population assessing planningdeveloping/maintainingimplementing and evaluating outcomes of same.
  • Documents educational process/patient information according to program/MDPH guidelines.
  • Communicates collaborates networks with and acts as a consultant to clients/significant others other members of the health care team and the community in order to ensure continuity of care and coordination of services and to provide needed resources to the community.
  • Writes and reviews protocols procedures and guidelines pertaining to clinical management

Qualifications

  • Bachelors in Nursing Required
  • Masters in Nursing Preferred
  • MI Registered Nurse (RN) Required
  • 5 years relevant experience in previous procedural nursing experience
  • AHA or ARC Basic Life Support (BLS) Required within 90 days
  • AHA Advanced Cardiovascular Life Support (ACLS) Required within 6 months
  • CRTOncology Certified Nurse ONCC Oncology Nursing Certification Corporation Required within 1 year
  • CRTPediatric Adv Life Support (PALS) preferred within 120 Days
  • 5 years of relevant experience previous nursing or CDE clinical experience Required
  • Prior teaching with patients of all ages Preferred
