Home Health Physical Therapist (PT)
A Home Health Physical Therapist is needed in Aurora Colorado.
Aurora is a thriving suburb of Denver offering a diverse community and outdoor lifestyle. This position is with a missiondriven home health organization focused on quality patientcentered rehabilitation care.
Salary and Benefits
- $90000.00 $120000.00 per year (payperpoint model)

Responsibilities
- Provide inhome physical therapy to adult and geriatric patients
- Develop and implement treatment plans and document progress
- Collaborate with care team to support patient outcomes
- EMR: HomeCare Home Base
- Specialties include geriatrics orthopedics and rehabilitation
- Patient visits scheduled based on clinician availability
- 25 points weekly to meet productivity
- Start of care 2 points
- All other visits 1 point
Requirements
- Active Colorado Physical Therapist license (or ability to obtain)
- Degree in Physical Therapy from an accredited program
- Home health experience preferred
- Strong communication and time management skills
- Ability to work independently in the field
