You will be updated with latest job alerts via emailThe Sepsis Coordinator is responsible for the development implementation and ongoing evaluation of Griffin Healths sepsis care initiatives. Serving as a clinical expert educator and performance improvement leader the coordinator will work across departments to reduce sepsisrelated mortality and enhance outcomes through early intervention and standardized care.
Lead and coordinate the hospitals sepsis program implementing best practices in line with current clinical guidelines.
Monitor and analyze SEP1 performance data; identify trends and improvement opportunities.
Conduct chart reviews and ensure documentation aligns with CMS measure requirements.
Collaborate with nursing quality infection prevention case management and leadership teams.
Provide ongoing education and support to clinical staff on sepsis recognition and treatment protocols.
Participate in root cause analyses and case reviews of sepsisrelated mortality or bundle noncompliance.
Stay current on national sepsis care standards and regulatory updates.
Assist with EMR optimization including sepsis alerts order sets and documentation workflows.
Present data and improvement plans to committees and leadership as needed.
Required:
Licensed Registered Nurse (RN) or equivalent clinical background
35 years of experience in acute or critical care settings
Strong knowledge of sepsis management and current clinical guidelines
Experience with quality improvement and data analysis
Preferred:
Bachelors degree in Nursing or related field
Experience with SEP1 measure abstraction or Core Measures
Familiarity with EMR systems (e.g. Meditech) and data platforms (e.g. QApp)
Certified Professional in Healthcare Quality (CPHQ) or similar credential
Strong leadership and communication skills
Analytical detailoriented and datadriven
Collaborative and selfdirected
Skilled educator and advocate for change
Passion for evidencebased care and continuous quality improvement
