Position Summary
The Intake Coordinator works under the direction and supervision of the Program Manager. He/she is responsible for all duties associated with the screening admission and referral of applicants to include proper written and software related documentation. Duties include medication monitoring and transportation of clients. He/she also participates in outreach efforts designed to educate the community and to identify potential clients.
Essential Functions
- Understands the mission organizational structure and policy and procedures of The Salvation Army. Works cohesively with supervisors.
- Coordinates the evaluation of applicants through screening documentation and facetoface interaction to determine if they are an appropriate fit for the programs. Coordinates admissions through contracted agencies. Provides client updates to contracted agencies as required. Provides applicable referrals when an applicant is determined to not be a proper fit.
- Updates and maintains a comprehensive spreadsheet of applicants waiting to enter the program and applicants referred to other programs due to ineligibility.
- Facilitates urinalysis and breathalyzer tests as directed. Immediately notifies supervisor when a resident test positive for drug and/or alcohol use.
- Issues and monitors resident medication per policy and procedure.
- Uses applicable software for reporting services provided and maintains backup documentation as directed.
- Ensures that clear and consistent information is communicated to applicants/new residents to include but not limited to: the waiting list the minimum standards House Rules requirements/expectations program fees and referrals).
- Transports clients to appointments as directed by supervisor
- Assists in outreach efforts designed to educate the community and to identify potential clients.
- Assists in preparation for inspections reviews internal and external audits.
- Performs other duties as necessary for the successful operation of the program.
- Maintain accurate time keeping records that are reflective of duties performed under different grant/contracts.
- Attends staff and program related meetings conferences and training as needed and required. Cooperates with the supervisor in any other special projects as needed.
- These duties may be changed in part or in full at any time at the Executive Directors discretion or as required because of program changes or needs.
Working Conditions
Ability to walk stand bend squat climb kneel and twist on an intermittent or sometimes continuous basis. Ability to grasp push pull objects such as files file cabinet drawers and reach overhead. Ability to operate computer fax and telephone. Ability to lift up to 25 lbs.
Minimum Qualifications
- Bachelors degree with major study in social work human services or related field and oneyear experience in residential services for adults. Applicable work experience in lieu of formal education will be considered.
- Experience working with people in recovery mentally ill and/or homeless individuals preferred.
- Must be able to pass a complete and satisfactory background check.
- Ability to drive and maintain a valid Class C California Drivers License. Able to pass a TSA Driving Test and maintain a clean MVR.
- Maintain a current TB test and CPR First Aid Certification.
Skills Knowledge & Abilities
- Excellent writing and verbal communication skills in English.
- Bilingual (Spanish) preferred but not required.
- Computer literate with a working knowledge of Microsoft Word and Excel.
- Ability to email fax operate copy/print/scan machine.
- Ability to work harmoniously with supervisors coworkers residents outside agencies and the public.
- Excellent organizational prioritizing planning and problemsolving skills.
- High ethics and integrity.
- Experience working for a Drug MediCal Program preferred.
