Job Description
- Creates discharge packets for patients going to skilled nursing facilities. Creating the discharge packet requires working with several disciplines to gather needed documentation such as the discharge summary MOLST medication administration record etc.
- Delivers durable medical equipment (DME) to the patients room for discharge and completing any associated insurance or charity paperwork.
- Confirms home care start of care.
- Delivers clothes suitable to the weather to the patients room for discharge.
- Completes patient screenings
Qualifications :
- High School diploma or GED is required
- CNA certification preferred
