The Department

The Mobile Assessment & Treatment Service (MATS) is a community and ambulatory service within the Health of Older People division of Alfred Home Acute and Community (HAC).

MATS overseen by Geriatricians is a multidisciplinary team of Medical Nursing Nutrition Pharmacy and administrative staff.

The service provides rapid clinical inreach to people living in residential aged care facilities RACFs and the community who are at risk of presenting to emergency departments for reasons that could be managed outside of the hospital setting. The MATS patient cohorts are the frail and elderly or those with severe disability in residential settings. Often MATS patients have a range of chronic conditions and conditions of aging.

The Role

MATS is looking for a dynamic and experienced Registered Nurse to join our team. The successful applicant will be motivated to optimise the health experiences of frail elderly patients living in residential care and the community setting.

The Outreach Nurse is responsible for:

  • Working within a collaborative multidisciplinary team framework to ensure best care whilst at times practising autonomously when providing that care
  • Conducting comprehensive and holistic systems assessments with astute ability to detect and identify clinical deterioration.
  • Delivery of safe patient centred care as determined by the multidisciplinary team and overseen by the Geriatric Consultants. Such care often includes (but is not limited to) administration of IV medications urinary catheter care wound assessments and management and supporting end of life care.
  • Evaluating and adjusting that plan of care in partnership with the patient and/or their decision maker their GP and the residential care staff and other members of the health care team.
  • This role works at the interface of acute/subacute hospital GPs and residential facilities to ensure integrated care for the person
  • The position requires excellent communication and documentation skills to foster effective relationships with internal and external stakeholders to the patients care.

This rewarding yet challenging role would suit RNs with significant experience in emergency acute general and/or geriatrichealthcare and those who share a passion for ensuring the very best care for older persons and their supportive networks.

Qualifications and Experience Required

Essential

  • Registration as a Registered Nurse with the Nursing and Midwifery Board of Australia via AHPRA
  • Relevant Clinical experience (preferred 5 years post graduation)
  • Demonstrated ability to work positively and flexibly with internal and external services
  • Demonstrated therapeutic interpersonal relationships with strong written and verbal communication skills
  • Commitment to continuing professional development
  • Current Victorian Drivers Licence

Desirable

  • Post graduate qualifications in gerontology nursing and/or palliative care nursing
  • Clinical experience in Community Wound Assessment & Management; Competence/credentialled in catheterisation for male/female IDCs & SPCs; Peripheral IV Cannulation
  • Recent clinical experience in General Medicine Acute aged care or Emergency Nursing
