Job Description
HIV Medical Case Manager
The Medical Case Manager is responsible for coordination of medical and related services for HIV individuals. The primary focus of the Medical Case Manager is to promote patient adherence with the treatment regimen prescribed by their HIV Provider including assistance in resolving barriers to HIV primary care. The Case Manager provides facetoface and nonfacetoface services both in the office and in the community.
Qualifications:Registered Nurse and/ or have a bachelors degree in social work psychology sociology or related field or bachelors degree in a nonsimilar field and two years experience in case management social work and/or a related field. English required. Bilingual in Spanish French or Haitian Creole preferred.
