Circadia Health is revolutionizing remote patient monitoring for postacute care and skilled nursing facilities. We are seeking a dedicated and experienced Nurse Practitioner to join our dynamic team. This role will be pivotal in assessing patients health status and providing care plans while utilizing cuttingedge technology to monitor and improve the health outcomes of our longterm care patients.
As a Nurse Practitioner with Circadia Health you will play a crucial role in our mission to enhance patient care through innovative solutions. You will be responsible for performing inperson patient assessments developing individualized care plans and collaborating with our interdisciplinary team to ensure the highest standards of care. Youll have the opportunity to travel to designated skilled nursing facilities within your region fostering direct engagement with patient care and onsite teams.
This is a perdiem position with flexible hours requiring weekday availability only.
Key Responsibilities
InPerson Comprehensive Assessments: Conduct detailed patient evaluations facetoface for current remotely monitored patients. Gather medical histories and assess current health conditions using both direct interactions and AIpowered insights to spot trends and predict issues.
Facility Visits: Travel to designated local skilled nursing facilities or patient homes to support patient care and the facilitys care team.
Personalized Care Planning: Develop tailored care plans. Define specific health monitoring parameters set realistic health goals and use predictive analytics to refine your clinical decisions.
Accurate Documentation & Compliance: Keep uptodate precise records of patient interactions assessments and care plans in the EHR system. Ensure all documentation meets regulatory standards and policies.
Collaborative Care: Work closely with the Skilled Nursing Facility clinical team Circadia physicians Circadia VRNs and other healthcare professionals.
Qualifications
Certified and licensed as a Nurse Practitioner in Michigan
Medicare participation and ability to have company bill for services on your behalf
Certification from ANCC (or equivalent) as an Adult Family Geriatric and/or Acute Nurse Practitioner
2 years of Nurse Practitioner Experience working with complex adult populations
Ability to practice independently with little or no supervision
Current MI Drivers License
Comfort using technology such as Google Suite multiple EMRs Slack AI transcription tools
Comfortable in a dynamic and always evolving start up environment
English required Spanish preferred
Compensation:
This is a paypervisit role. Compensation will be based on each completed and signed clinical note in our system.
