Job Description
Key Responsibilities
- InPerson Comprehensive Assessments: Conduct detailed patient evaluations facetoface for current remotely monitored patients. Gather medical histories and assess current health conditions using both direct interactions and AIpowered insights to spot trends and predict issues.
- Facility Visits: Travel to designated local skilled nursing facilities or patient homes to support patient care and the facilitys care team.
- Personalized Care Planning: Develop tailored care plans. Define specific health monitoring parameters set realistic health goals and use predictive analytics to refine your clinical decisions.
- Accurate Documentation & Compliance: Keep uptodate precise records of patient interactions assessments and care plans in the EHR system. Ensure all documentation meets regulatory standards and policies.
- Collaborative Care: Work closely with the Skilled Nursing Facility clinical team Circadia physicians Circadia VRNs and other healthcare professionals.
Qualifications
- Certified and licensed as a Nurse Practitioner in Michigan
- Medicare participation and ability to have company bill for services on your behalf
- Certification from ANCC (or equivalent) as an Adult Family Geriatric and/or Acute Nurse Practitioner
- 2 years of Nurse Practitioner Experience working with complex adult populations
- Ability to practice independently with little or no supervision
- Current MI Drivers License
- Comfort using technology such as Google Suite multiple EMRs Slack AI transcription tools
- Comfortable in a dynamic and always evolving start up environment
- English required Spanish preferred
