Position Overview:
The Hospice LPN will work closely with the interdisciplinary team to provide compassionate care to patients in their endoflife journey. This position will be responsible for administering medications monitoring patient symptoms and providing emotional support to patients and their families.
Key Responsibilities:
Administer medications and treatments as prescribed by the physician
Monitor patient symptoms and report any changes or concerns to the interdisciplinary team
Provide emotional support and comfort to patients and their families
Assist with personal care including bathing dressing and grooming
Document patient care and maintain accurate records
Collaborate with other healthcare professionals to develop and implement individualized care plans
Educate patients and families on endoflife care and support resources
Maintain a safe and clean environment for patients and staff
Adhere to all company policies procedures and regulations
Qualifications:
Current LPN license in the state of Illinois
Minimum of 1 year of experience in hospice or palliative care
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Compassionate and empathetic nature
Strong organizational and timemanagement skills
