Pharmacist St. Vincent de Paul Cincinnati (SVDP) has served residents of Greater Cincinnati for over 150 years with basic needs such as food medicine and homelessness prevention through its outreach center in the West End and 53 parishbased volunteer Conferences. SVDP provides holistic direct assistance and systemic change services while treating neighbors in need with dignity and compassion. Scope of Position: The Pharmacist is a clinical position that is essential to the mission of providing free prescription medication to lowincome neighbors in need. This position conducts activities related to the dispensing of prescription medications according to Ohio Board of Pharmacy rules and regulations. Provides excellent patient care through counseling and medication therapy management services. Leads activities of the pharmacy support staff including customer service medication inventory and troubleshooting as necessary. Job Responsibilities: - Process Fill Verify and Dispense Prescriptions
- Enter prescription data into QS/1 when necessary
- Assist with filling prescriptions when needed
- Verify all prescriptions for accuracy and compliance to policies and procedures
- Perform Drug Utilization Review (DUR) and assess medication adherence
- Counsel Each Patient Appropriately
- Review each medication dispensed with the patient addressing how and when patients are taking their prescription medications
- Assess patients response to therapy and address possibly side effects
- Address medication adherence concerns with patient and provide education as needed
- Provide Medication Therapy Management services when appropriate
- Refer complex counseling to the Clinical Pharmacy Team
- Provide Good Customer Service
- Ensure good customer service is provided to all patients in accordance with the organizations mission and policies
- Treat all clients with courtesy dignity and respect
- Inform the Lead Pharmacist or Pharmacy Director of customer complaints or staff concerns
- Lead the Activities of the Pharmacy Support Staff
- Ensure pharmacy technicians pharmacy interns and students are treated with respect and have adequate resources and training to complete daily tasks
- Ensure pharmacy support staff are assigned tasks to increase the efficiency of pharmacy operations
- Assist Clinical Pharmacist and Pharmacy Director in pharmacy training programs including PrePharmacy IPPE APPE and PGY1 Residency
- Ensure Adequate Medication Inventory
- Inventory medications and medical supplies regularly checking for sufficient quantities and expiration dates
- Alert pharmacy management to order specific prescription medications following an out of stock or fill when in issue
- Inform the Pharmacy Director of medication inventory issues and trends to address for quality improvement
- Troubleshoot Prescription Problems
- Contact medical providers for prescription clarification when needed
- Refer problems to Clinical Pharmacy Team call center when workload is high
- Inform Lead Pharmacist or Pharmacy Director regarding problems which are not readily resolved through daily workflow
Qualifications:Registered Pharmacist (RPh) in Ohio. Doctor of Pharmacy (PharmD) degree from accredited college of pharmacy. Immunization certified. At least 3 years of community pharmacy experience. Experience with pharmacy databases and Microsoft Word Excel and Outlook. Strong organizational interpersonal written and oral skills. Ability to work with people of diverse backgrounds in a faithbased setting. Work Environment:Staff member will primarily work in an office environment adequately lighted heated and ventilated at multiple locations within Greater Cincinnati.
