Home Health Transitional Care Nurse Practitioner (NP)
A Nurse Practitioner is needed in Rockford Illinois.
Rockford is a welcoming city known for its riverfront parks historic architecture and easy access to outdoor recreation. This opportunity is with an innovative healthcare organization dedicated to improving the quality of life for older adults through a personalized valuebased home health model.
Salary and Benefits
- Base salary: $105000 $135000 annually (based on experience)
- Annual clinical performance bonus:
- Less than 5 years of experience: $15000
- 5 years of experience: $25000
- Total earning potential: $150000 $160000 annually

Responsibilities
- Fulltime MondayFriday schedule
- Focus on postdischarge transitional care visits in a home health setting
- Patient caseload: 67 patients per day
- Responsibilities include:
- Medication reconciliation
- Social service followups
- General followup care and care coordination
- Collaborate closely with a Medical Director for clinical guidance
- Deliver highquality patientcentered care designed to reduce hospital readmissions and promote wellness at home
Requirements
- Master of Science in Nursing Doctor of Nursing Practice or Nurse Practitioner Certificate Program graduate
- Active and unrestricted Nurse Practitioner license in Illinois
- Active DEA license
- National board certification in one of the following:
- Family Nurse Practitioner (FNP)
- AdultGerontology Primary Care Nurse Practitioner (AGPCNP)
- Adult Nurse Practitioner (ANP)
- Gerontological Nurse Practitioner (GNP)
- US work authorization required
