Description

Humboldt Senior Resource Center is looking for a fulltime Home Care Scheduler to join the Redwood Coast PACE team. This position is Monday through Friday 8 am to 4:30 pm in our Fortuna location.

JOB SUMMARY: Under the supervision of the Home Care Manager the Home Care Scheduler coordinates and oversees PACE services provided in participant homes including scheduling contracted providers and employee CarePartners.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Maintains live scheduling calendar for Home Care services.
  2. Maintains master list of all participants and the current authorized inhome care hours.
  3. Schedules/reschedules inhome CarePartner assignments.
  4. Receives and responds to telephone calls from Home Care vendors and CarePartners; directs to Home Care Manager as needed.
  5. Assists Center Manager and Home Care Manager in coordinating and scheduling contractor and employee trainings.
  6. Assists home care team in monitoring Home Care hours and duties as provided by the participants Plan of Care.
  7. Reports significant participant condition changes to Home Care Manager.
  8. Follows agency policies for incident reports grievances and appeals.
  9. Communicates effectively with participants family members caregivers staff and visitors.
  10. Completes documentation accurately and in a timely manner.
  11. Complies with safety policies and procedures identifying and immediately reporting any potential or actual unsafe acts or conditions. Takes necessary measures to ensure a safe environment for oneself coworkers contractors participants visitors and others.
  12. Monitors personal safety and attends to good body mechanics. Ensures infection and exposurecontrol and is prepared in case of emergency.
  13. Continually seeks better ways for delivering services and communicating with participants.
  14. Demonstrates respect for and promotes participant rights including dignity selfdetermination access to care confidentiality and independence.
  15. Effectively collaborates with staff peers and contractors to meet PACE goals and further success.
  16. Complies with all policy and procedures.
  17. Demonstrates proficiency in delivering agespecific care.
  18. Consistently meets or exceeds quality assessment and performance improvement targets.
  19. Participates in oncall coverage in conjunction with the Home Care team.
  20. Acts in a manner consistent with agency values.

Qualifications

EDUCATION &/or EXPERIENCE:

  1. High school diploma or GED required.
  2. One (1) year working with a frail or elderly population preferred.

PRESCREENING REQUIREMENTS:

  1. Requires clearance of a DOJ and FBI criminal history background check
  2. Preemployment physical and PPD skin test.
  3. COVID19 and Influenza (flu) vaccines preferred.

  1. Customer service skills necessary to work and communicate with diverse populations and to interact and maintain relationships with program stakeholders in a friendly courteous and professional manner.
  2. Excellent interpersonal interactions that result in team building and contribute to effective work relationships successful negotiation and conflict resolution. Demonstrates effective social interaction with coworkers management participants and community contacts.
  3. Ability to communicate verbally and written form in a friendly courteous and professional manner.
  4. Ability to collect data identify pertinent information and prepare reports and presentations.
  5. Highly organized and able to multitask.
  6. Ability to work independently and with minimal supervision.
  7. Maintains excellent attendance and punctuality records.
  8. Ability to work in a constantly changing environment with individuals of various backgrounds and abilities.
  9. Maintains a positive attitude and balance in relationships with others in complex interpersonal situations.
  10. Proficiency in Microsoft Word Excel PowerPoint Outlook and relevant computer programs and software (e.g. electronic health record software). Basic knowledge of webbased applications such as timekeeping training and email programs.

PHYSICAL REQUIREMENTS:

  1. Ability to sit at a desk and interact with a computer screen for extended periods of time.
  2. Hand and arm strength sufficient to operate a keyboard for several hours each day.
  3. Physical abilities sufficient to move between different staff work areas communicate with staff and the public operate a computer produce reports talk on the telephone and travel to other Agency worksites.
