Job Description

Overview

The Sorenson Center for Clinical Excellence (SCCE) is seeking a Mental Health Clinician who will work with interdisciplinary clinics in the Sorenson Center including the Dementia Caregiver Clinic and oversee outreach activities for the SCCE. Located on the main campus in Logan Utah this is a 1.0 FTE 12month fiscal year position. This position is not eligible for remote work. This position is contingent on funding and a successful background check.

The hired individual will serve as the care coordinator for the Dementia Caregiver Clinic and will also assist other clients in accessing appropriate services in the SCCE with a particular focus on assisting clients in need of multiple services. In addition the hired individual will oversee the activities of the Sorenson Centers community outreach program and the Sorenson Centers social media platforms. The clinician is expected to engage in outreach activities through the state of Utah including supporting the growth and delivery of services via telehealth throughout the state. The clinician is also expected to engage in clinical activities and assist in facilitating educational seminars especially those that support the interdisciplinary focus of the Sorenson Center.

Responsibilities

The Mental Health Clinician is responsible for the supportive functions and internal/external coordination of Sorenson Centers community programs in conjunction with the Operations Manager and the Executive Director. The clinician will report to the Executive Director. The clinician may supervise graduate interns and undergraduate practicum students.

Specific responsibilities include:


Case Management and Clinical Services (70%)

  1. Serve as lead clinician and coordinator on Dementia Caregiver Clinic (DCC) team. In this role the clinician will conduct initial phone intakes of individuals interested in services. The individual will also conduct consultations with clients and serve as the coordinator for the DCC team.
  2. Oversee singlesession consultations. The clinician will oversee the SCCEs single session consultation services including coordinating with other clinicians and clients.
  3. Provide case management for interdisciplinary clients or clients with complex needs assisting clients in coordinating services in the SCCE and broader community. In this role serve as a member of the Interdisciplinary Assessment team as needed.
  4. Positively contribute and interact as a team member in the development growth and sustainability of the SCCE telehealth services with a focus on behavioral health. Engage in outreach throughout the state to increase reach of the SCCE telehealth services.
  5. Provide and/or oversees triage of clients seeking behavioral health services who need assistance in accessing the most appropriate services.
  6. May train and direct lowerlevel staff student employees volunteers and/or graduate students in clinical and outreach work.
  7. As time allows provides oneonone therapeutic services to clients based on client needs and provider expertise. Conforms to insurance and accreditation standards as a licensed professional.

Outreach/Education Coordination (25%)

  1. Design coordinate and implement innovative community outreach initiatives with a focus on outreach to rural / unserved communities in Utah and promotion of the SCCEs telehealth services.
  2. Colead the interprofessional education training curriculum for students who have practicum placement in the Sorenson Center. Teach seminars and coordinate other faculty/staff in their roles with this training.
  3. Design content and postings for the SCCE social media platforms.

Other administrative Duties (5%)

  1. Maintain current knowledge of trends and issues in digital health and related topics and share this knowledge with other SCCE providers.
  2. Perform other duties as assigned to support the SCCE and its mission.
