Director of Case Management
Director of Case Management is needed in Oviedo FL.
Titan Placement Group invites you to explore an opportunity in Oviedo FL. Oviedo is a hip affluent city northeast of Orlando in Seminole County known for its historic residential homes as well as a curious population of chickens that roam the downtown area. Starting from a post office near Lake Jesup Oviedo was named after the city of Oviedo in northern Spain to honor the Spanish heritage of the state of Florida by an early settler of the area Andrew Aulin.
This position is with a progressive financially stable Hospital System.
Salary and Benefits
- Hourly rate: $102107.20 Mid $127587 Max $153088
- Target Bonus: 15% (Based on performance)

Responsibilities:
- Directs and evaluates departmental operations including the case management model staffing (skill mix and FTEs) use of information technologies onboarding and staff competencies to achieve performance and quality objectives.
- Allocates resources to effectively staff the department and meet productivity and quality goals.
- Responsible for oversight of CM core functions and practice.
- Ensures adherence to care coordination and discharge planning processes ensuring timeliness quality and proper documentation.
- Works with the Facility CFO and Division CM Leader to achieve established goals and expectations.
- Assesses and improves the departments performance by evaluating operational processes monitoring performance through analyzing data and implementing sustainable performance improvement activities.
- Ensuring compliance with policies and SOPs.
- Establish working relationships with key stakeholders to include CMO CNO ancillary service leaders Ethics & Compliance and Legal.
- Performs other duties as assigned.
- Practices and adheres to the Code of Conduct and Mission and Value Statement.
Requirements
- Must have at least 2 years of Recent ACUTE CARE Case Management experience with 57 years of clinical experience.
- Bachelors degree required
- Masters degree in Nursing Health Administration or Business Administration preferred
- Registered Nurse (RN) licensure (Cannot be LCSW)
