Job Description
Spero Health is seeking an Addiction Medicine Nurse Practitioner to join our team in Warren!
This position is for an Addiction Medicine Nurse Practitioner with the ability to prescribe buprenorphine to join our team to fulfill its mission of Saving Lives Instilling Hope and Restoring Relationships to those suffering from substance use disorder. We provide local and affordable outpatient care primarily to those suffering from opioid addiction and we operate as one of the largest Office Base Opioid Treatment (OBOT) providers in the country.
Spero Health utilizes an innovative evidencedbased integrated care model that combines both physical and behavioral healthcare services under one roof to treat the whole patient. We offer medicationassisted treatment along with counseling and case management within our facilities in order to bring the most comprehensive level of care to the patients that we serve while our communitybased approach to treatment allows patients to continue working and caring for their families as they overcome addiction.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provide initial evaluation diagnosis treatment and ongoing support for individuals with complex addiction and substance related health conditions.
- Complete full and appropriate documentation to support professional billing for services.
- Provide education regarding illness and prognosis to assist with clarification of patient and family goals of care and support development of a comprehensive care plan.
- Interpreting test results to diagnosis any health issues and provide feedback to patients.
- Educating and counseling patients regarding medication compliance.
- Work with an interdisciplinary team of professionals to administer the best care for those patients that need not only medical therapy but behavioral health services as well.
- Patient followup visits regarding patients progress with medication and/or treatment.
- Contacts patients PCP and/or specialist(s) when necessary.
- Evaluates lab results and provides consultation with APPs regarding abnormal results.
- Reviews and oversees progress and treatment plans including making any adjustments.
- Maintains required licensure certifications and CME credits.
MINIMUM QUALIFICATIONS:
- Active and unrestricted medical license as a nurse practitioner within the applicable state.
- Experience providing addiction medicine services for both medically and socially complex populations; Experience managing complex substance use disorders.
- Ability to develop relationships with network and community physicians and other providers.
- Current active DEA licensure and prescriptive authority in state(s) where applicable.
- Ability to work onsite with diverse care teams in a variety of settings.
- Effective time management and communication skills.
- Computer literate and experience with Electronic Medical Records.
