Job Description
Nurse Practitioner (NP)
A Nurse Practitioner is needed in Louisville Kentucky.

Salary and Benefits
- Base salary: $140000 annually
- Annual bonus potential: Up to $10000 in the first year (increases with partnership opportunities)

Responsibilities
- Fulltime MondayFriday schedule
- Clinic hours: 7:30 AM 5:00 PM (last patient scheduled at 4:15 PM)
- Outpatient primary care setting (100% outpatient)
- Patient volume: 18 patients per day
- Small patient panel: Maximum of 450 patients
- Focus on senior patients with 35 chronic conditions
- Provide preventive holistic and valuebased care
- Supported by a dedicated team including your own Medical Assistant and Scheduler
- Administrative time included in the daily schedule
- No nights no weekends and no hospital rotations
- Access to inhouse consulting specialists including cardiology podiatry acupuncture tai chi yoga and nutrition services
- Doortodoor transportation available for patients
Requirements
- Board Certified Family Nurse Practitioner or Geriatric Nurse Practitioner
- Active and unrestricted Nurse Practitioner license in Kentucky
- At least 2 years of primary care outpatient experience required
- Passion for serving the senior population with a valuebased care model
