Job Description
- Provide safe and effective patient care services while promoting safe and effective access to medication therapy.
- Provide patientcentered care through interaction and facilitating effective communication with patients caregivers and stakeholders.
- Participate in the identification and implementation of medicationrelated interventions for a patient population (population health management). Conduct pracrice advancement projects.
- Demonstrate leadership skills that contribute to departmental and/or organizational excellence in the advancement of pharmacy services.
- Demonstrate leadership skills that foster personal growth and professional engagement.
- Provide effective medication and practicerelated education.
- Provide professional and practicerelated training to meet learners educational needs.
- Complete all additional activities required and outlined by individual Pharmacy Residency Program
- Required Bachelors Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy. Completion of an ASHP accredited PGY1 residency.
- Experience gained through completion of a PGY1 residency program. Required
- LICClinical Pharmacist STATEMI State of Michigan 90 Days required
- LICPharmacist Controlled Substance STATEMI State of Michigan 90 Days required
- CRTBasic Life Support (BLS) AHA American Heart Association 180 Days required Or
- CRTBasic Life Support (BLS) ARC American Red Cross 180 Days required Or
- CRTPediatric Adv Life Support (PALS) AHA American Heart Association 180 Days required Or
- CRTAdv Cardiovascular Life Support (ACLS) AHA American Heart Association 180 Days required
