Interested in working to improve the overall wellness of Grand Rapids homeless and lowincome residents regardless of resources or ability to pay This is perhaps our most important service as the health of any community depends on all its residents health.

The Corewell Health Medical Group Community Medicine Clinic offers fullservice primary care with after hours oncall nurse triage for urgent needs. We have an integrated behavioral health model to serve populations seeking to access mental health services in the community. Our goal is to help develop patientcentered plans of care. We also provide support and education with a nurse case manager who specializes in the management of patients with chronic disease.

We support individuals and family members who are struggling with mental health disorders including substance use disorder and addiction and help connect patients with substance use disorder services in the community. We provide LGBTaware care with onsite transgender healthcare. Additionally we work closely with homeless populations and newly arrived refugees.

Corewell Health Community Medicine Clinic is looking for a skilled physician to join their busy practice caring for patients across the generational spectrum located in the heart of Grand Rapids.

Qualifications

• Required bachelors degree
• BE/BC Family/Internal Medicine
• LICPhysician (MD) STATEMI State of Michigan Upon Hire required Or
• LICOsteopathic Physician (DO) STATEMI State of Michigan Upon Hire required
• CRTDEA Registration STATEMI State of Michigan Upon Hire required
• CRTBasic Life Support (BLS) AHA American Heart Association 120 Days required Or
• CRTBasic Life Support (BLS) ARC American Red Cross 120 Days required
• CRTAdv Cardiovascular Life Support (ACLS) AHA American Heart Association If required by specialty or stress test performed 120 Days required
