Position Overview:
As a Licensed Practical Nurse (LPN) youll provide highquality nursing care in the home setting under the supervision of a Registered Nurse (RN). Youll help deliver curative rehabilitative palliative and preventive care while supporting patients on their path to recovery and independence.
Key Responsibilities:
Provide skilled nursing care under the supervision of an RN
Deliver individualized care as outlined in the patients plan of care
Monitor patient progress and report changes to the RN and physician
Educate patients and families on treatment and selfcare techniques
Ensure accurate and timely clinical documentation
Collaborate with the interdisciplinary team to ensure excellent care coordination
Comply with all agency policies and clinical practice standards
Qualifications:
Active LPN license in the state of Florida
Graduate of an accredited nursing program
At least 1 year of nursing experience (home health or med/surg preferred)
Current CPR certification (BLS)
Reliable transportation with a valid drivers license and auto insurance
Excellent communication organizational and problemsolving skills
